IF you're playing on xbox live than you just go to 'host private match' and invite your friends to play with you. Matchmaking picks a random host so basically no one knows if they are the host or not.
IF your playing on PC you need to pay a server company to 'rent' you space on one of their giant servers so you can have your own server and edit all the options of that server ( Gametype, Time Limit etc. ). However, you could also host it your self off of your own computer or better yet a linux based machine... but that's another story.

I think you have to be the first one on the board after you pick a game. It will say "Waitng for other players to join" or something close to that.
If you have mates online and invite them to join a new game and they accept you become the host. Anyone else online wanting to play the same mission (domination, sabotage) will be put into the game to make up the numbers.
i know the feeling, but you dont even know ur actually the host anyway, not as if you can choose a decent map anyway!
