No, you don't have to know the host but you should introduce yourself soon after you arrive to the party (and not as you are leaving). You should know ahead of time what the dress (code) is. If you are not sure then you must ask. You don't want to show up looking like someone who wasn't invited.
Kinda depends on what kind of party. For most casual events you don't need to know the host. If you are tagging along with a friend I would say it is good form for your friend to ask the host beforehand if its ok that you come. If its like some kegger or something then no you don't need to know the host. If its a formal type party like a wedding reception or something you should probably know the host.

that is still fine as long as you know somebody you won't feel leftout. well if you aren't invited by host and got in somehow it's still fine, just enjoy the party and if you feel like you don't belong there or your being treated somewhat hostile it's better to leave the place or don't go
